Player Forms
New Starz Players - or Returning Starz Players (annual registration)
Each season, returning Starz players and all new Starz players must complete the following forms:
1. Annual Registration & Health form
2. Starz Financial Agreement
3. Player/Parent Code of Conduct
Please click on the form packet (to the right) to print out a copy. Bring the completd and signed forms to the Starz kick-off meeting in September (meeting date TBA):

Trial Players
Occassionally, a team or coach may have the need for an additional player during a tournament, or when a player is considering joining the Starz program mid-season. A potential new ("trial") player is limited to participating in two tournaments and/or two calendar weeks of Starz practice sessions before she will be expected to join the team or cease participating with the Starz organization.
